HRL engineers use advanced techniques and standardised procedures to study failed plant or components to determine the root cause of the failure and to identify and evaluate the factors that may have contributed to the failure or incident. 

These investigations inform a range of subsequent activities including:

* Evaluate repair options
* Revise operational procedures
* Recommend de-rating of equipment
* Develop remedial actions
* Improve design
* Plan inspections and program maintenance
* Identify liability and culpability
